Driver's Medical Report

ScotiaMed is currently running hours specifically for the Driver's Medical Form completion:

• Tuesdays from 10:30am to 12:30pm

• Thursdays from 10:30am to 12:30pm

Appointments can be booked in advance by phone (Ext. 1, open from 2:00pm to 4:00pm), in person, or by walk-in on the day of (appointments not guaranteed if walking in).

You must file a completed Driver's Medical Examination Report with Access Nova Scotia when you first apply for a class 1,2,3,4, at license renewal, and annually after age 64. Medical information may be requested at the Registry's discretion when applicable. 

MSI does not insure Driver's Medical Reports; there is a $150.00 fee for physician completion (cash/debit only). We will provide a receipt of payment for patients to submit to private insurance (if applicable). We do not direct bill to third-party insurance companies.
